Instructions – By Space Request Type

The following Space Request types are initiated through the Facilities and Planning – Work Order Admin System.

Requesting Space, Remodel/Modifications/Construction, 110/210 Classroom Modifications, and Non-maintenance Painting, Carpeting or Blinds must be approved in advance of the request being submitted online. These requests are then to be submitted online by the ‘Authorized SPAC Form Submitters’ only.

See request type below for all required forms and documents to be submitted to the appropriate ‘SPAC Form Template Approver‘ prior to being entered online.

Requests must be approved in advance of the request being submitted online. These requests are then to be submitted online by the SPAC form approvers and designees only.

A department or university entity seeking existing available space shall submit their request to the appropriate approver or designee using the SPAC Request Form Template. The SPAC Request Form is then completed and submitted by the Authorized SPAC Form Submitter or designee.

Provide a brief overview of the request in the ‘Description’ field
Add a more comprehensive explanation in the ‘Additional Information’ field

Sketches or additional information you would like reviewed, can be attached when submitting the SPAC Request Form.

SPAC Request Form Program Space Move Detail SPAC Request Form Template

This includes office moves within a department or program in space previously assigned between staff/faculty, new employees and those no longer employed at EWU.

To notify C&P of new staff or changes to staff/faculty office locations – start by submitting a SPAC Request Form and completing the Individual Space Move Detail Form. This request type does not require SPAC approval.

Select the Request Type of ‘Internal Departmental Office Moves’

SPAC Request Form Individual Space Move Detail

Requests must be approved in advance of the request being submitted online. These requests are then to be submitted online by the SPAC form approvers and designees only.

A department or university entity seeking to change the intended use of a FICM 110 or 210 classroom or would like to change from General Use Classroom to General Departmental Classroom, shall submit their request to the appropriate approver or designee using the SPAC Request Form Template. The SPAC Request Form is then completed and submitted by the Authorized SPAC Form Submitter or designee.

Requests must be approved in advance of the request being submitted online. These requests are then to be submitted online by the SPAC form approvers and designees only.

A department or university entity seeking to ‘Remodel/Modifications/Construction’ of a space shall submit their request to the appropriate approver or designee using the SPAC Request Form Template. The SPAC Request Form is then completed and submitted by the Authorized SPAC Form Submitter or designee.

If a change in paint, carpeting or blinds is requested outside of the standard replacement cycle, this expense is incurred by the requesting department.

A department or university entity seeking ‘Non-maintenance painting, carpeting or Blinds’ shall submit their request to the appropriate approver or designee using the SPAC Request Form Template. The SPAC Request Form is then completed and submitted by the Authorized SPAC Form Submitter or designee.
